📚 Faith Formation Registration
This form is required for all children participating in Tuesday Classes or Home Study. Please complete it once per child, every year.

Medical forms are required for each child once every 3 years, unless information has changed. If you’re unsure whether one is on file, feel free to email us!

Once completed, forms can be submitted via email or dropped off in person.
